How To Request Proxy Access in MyChart

Here we address how a patient can request proxy access through their MyChart account. If a parent has their own MyChart account, they can request access to minor's record, or you can grant another adult access to view their own MyChart account. It is good practice to assist the proxy and the patient in signing up for MyChart during an office visit.

Important note: If the proxy is not a current MUSC patient, the front desk staff will create an MRN for proxy use and can send an activation link to them via email. If you are unable to locate the proxy's MRN, please request that a member of the front desk staff create the MRN.

Parent with Active MyChart: Requesting Child Proxy

  1. Log into MyChart as the adult who is requesting access to a minor's record.
  2. Open Your Menu and search for Proxy, or scroll down to Request Proxy Access (Child Under 16).
  3. Complete the form and click the Submit Request button.

Important note: Once this form is submitted, you should hear back from MUSC Health's medical records team within three business days. Once granted proxy, please remember you will log into your own account and then be sure to switch accounts to view the Proxy account info.

Patient with Active MyChart: Granting Access to Another Adult

  1. Log into your MyChart account and open Your Menu and search for Proxy or scroll to the Share My Record section.
  2. Select the Friends and family access button. Share My Record Options
  3. Click the +Invite Someone button.
  4. Complete the form and click the Send Invite button. The person you invited will be able to log into MyChart or use the Create Your Account link in the email.

 

Adult Patient Without a MyChart Account Signing Up

There are several options for MyChart signup:

  • Clinic staff can help you sign up directly before or after your visit.
  • Clinic staff can provide you with a MyChart activation code on your After Visit Summary or on a billing statement.
  • You may receive a text message or email with an activation code when you come in for a visit.
  • You may be able to use self-signup online to create a MyChart account by matching your information against what is on file in your medical record or with third-party identity verification.
